Facility Use Application

Applicants take full responsibility for injury and property damage. Fill out the form and submit it to the MUMC Church Secretary.

Gym Rules  & Liability Waiver

Keep the Gym Looking Good!
• Keep gym clean and picked-up
• No heavy carts or equipment
• No skates or scooters
• No food, gum or liquids – except water with a cap or cover
• Gaffer or painting tape only
• Use gym at your own risk
